PLEASE READ THROUGH THESE INSTRUCTIONS COMPLETELY BEFORE MOVING FORWARD.

Your loan has been approved. You are now required to complete and upload the following documents: (PDF format preferred)

• Signed Promissory Note with the City of Pleasanton
• Signed Agreement for Automatic Deduction (ACH) of installment payments
  Please disregard instructions to mail to City Finance Department. The Agreement for ACH must be uploaded here.
• Copy of voided check (for the account noted in the ACH which will be used for repayment)

How to download and upload completed documents:

1. Scroll down to Documents.
2. Click on each document link to print. 
3. Sign the Promissory Note and scan as a PDF file. 
4. Complete the Agreement ACH form; make sure you sign this and provide an email address. Do not mail this form to the City Finance Department. Scan and save as a PDF file. 
5. Download the sample voided check file to view an example. Void a check from the account you designated in the ACH form. Scan and save as a PDF file. 
6. Click on Upload Contract Documents. A pop-up window will appear where you can upload the three completed documents. Upload one at a time. You can rename the documents here so they do not have the same filename as the documents you downloaded. Ex. Promissory Note Signed.
7. After you upload each document, they will appear on the list in the window. 
8. Once you are done uploading, close the window. Refresh the page by clicking Refresh Page at the top of the application. Your newly uploaded documents should now be listed.
9. You should see SIX documents; the three documents that you downloaded and the three completed/signed documents that you uploaded.

Signatures

1. Confirm that you have downloaded the provided documents and uploaded the completed/signed documents by placing your initials in the box titled Sign here to accept next to each uploaded document. You will initial six boxes.
2. Under Signatures, type your full name in the box titled Sign here to accept to confirm that you have read the Contract below.

Tip: Refresh Page often to see that your page has been updated.

You may save this page as a PDF by clicking Save Contract as PDF. You may now log out of your application. Once we receive your required documents, it will take up to 2 weeks to receive the loan check via mail. If you have any questions, please contact econdev@cityofpleasantonca.gov.
