Attestation

1. On September 22, 2021, the King County Executive issued Executive Order #ACO-8-28-EO, which requires workers of King County contractors to be fully vaccinated before beginning work if the workers will be: 1) providing on-site services on county property or in county facilities, or 2) providing services requiring interaction with the general public. If you are awarded a contract as a result of this RFP, you will need to submit an Evidence of Vaccination – Attestation form to King County before you can begin performing services. If you have concerns about meeting this requirement, please reach out to the Procurement Lead. Organizational Size
The following questions will help us determine if you are eligible for technical assistance.

2. What was your organization’s annual operating budget the past fiscal year? If you are a satellite office of a larger organization, please provide the budget for your stand-alone, fiscally-separate entity.

3. How many full-time equivalents (FTE) does your organization employ? Please calculate FTE by converting hours worked by part-time employees into full-time equivalent hours (e.g., two employees working 20 hours per week equals one FTE).

Background

1. Please provide an overview of your organization’s mission and services. Please include information about your organization’s experience and expertise serving seniors.

Project Model

2. Please describe your project model. In your response, please address: the main project components and goals; how the project will support senior participants to build social connections and reduce social isolation and/or loneliness; who the project will serve and how you will do outreach to engage participants; and how this project model reflects the needs and interests of the participants.

3. If your organization currently receives funding for social engagement programming for seniors from the VSHSL, please explain how your proposed project is meaningfully distinct from your services already funded through the VSHSL. If your organization does not currently receive this funding, please indicate N/A.

4. Please provide a project timeline that summarizes the main activities of the August 2022-December 2023 program period (ex: project start-up activities if applicable, significant project milestones, outreach activities, etc.).

Equity and Social Justice

5. Please describe how your organization will work to advance equity in your proposed project, for example, by: targeting seniors with the greatest needs; prioritizing accessibility and working to remove barriers to participation; creating inclusive and participatory environments; and/or addressing systemic issues that contribute to social isolation and loneliness.

Budget and Fiscal
The Department of Community and Human Services (DCHS) has an interest in ensuring organizations have equitable access to public funds. DCHS also has a responsibility to be a good steward of public funds, foresee risk when possible, and work with entities to mitigate that risk. To that end, we would like to understand your organization’s fiscal landscape. Note that answers to these questions may not, alone, exclude an organization from eligibility for funding. It is our goal to work with organizations to ensure access to funds and manage risk together.

6. Please provide a proposed project budget (See the provided budget template in Attachment C).

7. If your organization has ever contracted, managed or accounted for public funds, please describe that experience. If not, please indicate N/A and respond to the next question.

If your organization has not ever contracted, managed or accounted for public funds, please describe how you intend to manage public funds should you be awarded VSHSL funds. Please include in your answer if your organization would seek a fiscal agent and/or if your organization would seek technical assistance (neither is required).

9. Describe any audit findings or concerns during the last 24 months as well as the resolution of each (this can be a financial statement audit, federal single audit, monitoring site visit by King County or another federal, state or local agency). If there was a summary report of the outcome produced for any audit, please upload this report to the Required Documents tab in ZoomGrants.

10. Describe your organization's financial management system. How do you establish and maintain general accounting principles, sound accounting systems and internal controls? If your organization is new or in its early stages, describe how you intend to structure these systems, what in-house experience exists in your organization to establish these systems, and what, if any, assistance you are seeking to ensure these systems are established.
