The Durham County Alcoholic Beverage Control Board has established a grant program for the purposes prescribed in Chapter 18B-805(h) of the North Carolina General Statues. Priorities for the use of these funds shall be spent for:
the treatment of alcoholism or substance abuse, or
for the education on alcohol or substance abuse projects that target youth and
to promote the intervention among youth.
The funds shall be used for the benefit of Durham County residents.

3. Does your organization meet the Durham County ABC Board’s eligibility criteria for nonprofit entities seeking grant funding? As described in the previous tab.

Is your organization prepared to submit required documentation verifying nonprofit status, including the two most recent years of tax paperwork (e.g., IRS 990 forms or state nonprofit filings) New organizations or those unable to submit two years of tax documentation may provide an explanation of their circumstances later in this application.

4. If you are applying for funding for a specific project or program, enter the following information:

Be sure to identify the needs or specific problems that will be addressed - include the population served, and the goals of the program. b. Describe the proposed activities of the project., including a timeline c. Describe a plan for measuring your programs success. The plan should indicate the anticipated changes or results if the program objectives are achieved. Clearly identify how this program/project addresses alcohol or substance abuse in Durham County.
5. If you are seeking funding for operating expenses, enter the following information: a. Provide detail as to how the grant will allow the organization to continue to support or expand current programs. Include the population that your organization serves. b. Describe the anticipated use of the funds, including a timeline. c. Describe a plan for measuring your organizations success. Clearly identify how this program/project addresses alcohol or substance abuse in Durham County.
6. Please acknowledge that you have read and understand the following: The Durham County Alcoholic Beverage Control Board, in its sole discretion, retains oversight authority of the Grant Program, including but not limited to the following remedies: a. Upon a finding of any material misrepresentation in the Grant Application process or upon a finding of mismanagement of grant funds after an award has been granted, the Durham County Alcoholic Beverage Control Board reserves the right to terminate any awarded grant and seek reimbursement of any funds awarded, and/or to withhold any additional funds that might otherwise be due under Section V; b. The Durham County Alcoholic Beverage Control Board may request a site visit from any grantee in order to allow the Board to gauge the proper management and effectiveness of the grantee program. If, in its discretion, the Durham County Alcoholic Beverage Control Board finds that a grantee refuses or is non-cooperative in facilitating a site visit, the Durham County Alcoholic Beverage Control Board reserves the right to terminate any awarded grant and seek reimbursement of any funds awarded, and/or to withhold any additional funds that might otherwise be due under Section V; c. Upon a finding of any material misrepresentation in the Grant Application process or upon a finding of mismanagement of grant funds after an award has been granted, or if the grantee refuses to cooperate with a requested site visit, neither the applicant nor its principals shall be eligible to apply or participate in the Grant Program for one calendar year.

For non-profits, provide a copy of the 501(c)(3) federal tax exemption letter of determination and, if applicable, evidence of your organization's authority to operate under the 501(c)(3) of another organization.
Provide an audited financial statement, or a copy of filed tax form if the organization does not have an audit performed. These documents must not be more than 2 years old.
For public schools, provide a letter on official letterhead providing the nine-digit tax identification number of the applying public school and signed by the senior officer.
