THIS APPLICATION IS ONLY FOR THOSE WHO LIVE IN THE CITY OF DALLAS WITH A 752_ _ ZIP CODE AND HAVE NOT PREVIOUSLY RECEIVED ANY ASSISTANCE FROM HARMONY CDC OR ANY OTHER AGENCY DISTRIBUTING CITY OF DALLAS FUNDS! 

HOW TO APPLY:

In the upper right corner click on “New ZoomGrantsTM Account” and create a Zoomgrants login with a valid email address and password.  SAVE YOUR LOGIN INFORMATION SO YOU CAN MAKE CHANGES/CHECK UPDATES ON YOUR APPLICATION.  The email address you use to create your account will be the one that you receive all correspondence regarding your application.  YOU ARE RESPONSIBLE FOR CHECKING THIS EMAIL ADDRESS FOR UPDATES.

Once your account is created you can start your application.  You must answer every question or your application cannot be submitted.  In addition, you must upload all the required documents or your application will not be considered complete.  AN INCOMPLETE APPLICATION WILL NOT BE REVIEWED.  You are able to save and come back to the application at a later time.  To do so, you will need to use your login information to access your application.  YOU CAN ONLY SUBMIT ONE APPLICATION SO DO NOT LOSE YOUR LOGIN INFORMATION.

Your application is only considered complete if you have answered all the questions AND uploaded all required documentation.  Your application will not be processed until it is complete.  Once your application is complete, you need to allow up to 3 business days for your case manager to contact you.

HOUSEHOLDS MUST BE ABLE TO PROVE THEY ARE AT DIRECT RISK OF HOMELESSNESS.  YOU WILL BE REQUIRED TO SUBMITTED PROOF OF INCOME, PROOF OF HOUSING, PROOF OF HABITABILITY OF HOUSING, AND OTHER DOCUMENTATION AS ASSIGNED BY YOUR CASE MANAGER. 

HOUSEHOLDS MUST SHOW THEY ARE UNABLE TO PAY THEIR RENT/UTILITIES BECAUSE OF DIRECT IMPACT FROM COVID-19.  WE ONLY HAVE FUNDS FOR THOSE THAT HAVE BEEN IMPACTED BY COVID-19. 

You are REQUIRED to submit documentation to prove you are unable to pay your rent/utility because of COVID-19 due to at least ONE of the following reasons:

REASON 1. Business closure – your place of employment closed because of COVID

REASON 2. Non-essential job – your hours were reduced, or you lost your job due to COVID

REASON 3. School closure – your children’s schooling was moved to online and you are the one responsible for teaching them from home

REASON 4. Self-isolation/ self-quarantine/ illness – you contracted COVID, or were forced to quarantine due to the possibility of contracting COVID, or your mental health has been impacted by COVID which prevented you from working.

The reason you select as your COVID hardship will determine the supporting documents you need to submit as proof, supporting documents include:

REASON 1 & 2: Business Closure and Non-Essential Job Loss –  MUST SUBMIT ONE OF THE FOLLOWING

    1. Unemployment Determination on Benefits award letter with up-to-date payment history
    2. Last paycheck with full pay (before COVID impact) and current paycheck with decreased pay (after COVID impact)
    3. Letter from employer stating loss of job/loss of hours due to COVID

REASON 3: School Closure – MUST SUBMIT ONE OF THE FOLLOWING

    1. Proof of virtual learning platform that contains child’s name
    2. Letter from school stating child is enrolled in virtual learning
    3. School closure email or letter

REASON 4: Self-isolation, self-quarantine, or illness – MUST SUBMIT ONE OF THE FOLLOWING

    1. Proof of positive COVID test
    2. Letter from job stating you were required to be placed in quarantine
    3. Hospital release form stating you were in the hospital due to COVID
    4. Medical paperwork from doctor stating high risk for COVID
    5. Medical paperwork from doctor regarding mental wellness was impacted due to COVID

If you cannot provide proper proof of your reason for COVID hardship then you do NOT qualify for our funding and your application will be denied.

Rent funding is restricted by the following requirements:

  1. Applicants must be in the City of Dallas with a 752_ _  zip code 
  2. Applicant must be below 50% AMI 
  3. Award amount is based on FMR and cannot exceed $1500
  4. Applicant must be housed and at immediate risk of homelessness
  5. Applicant must not have sufficient resources or support networks immediately available to prevent them from becoming homeless 
  6. Assistance can NOT be provided for those living in public housing or Section 8 housing vouchers

Utility funding is restricted by the following requirements:

  1. Can only award a maximum of $1,500 per month cap as above
  2. Can include one-time payment of utility arrears (and late fees) which counts toward the 6 month cap 
  3. Can only be used for electric, gas, water, trash, sewage

We are only able to pay via check and all checks will be made payable to the landlord.  We will NOT issue any checks directly to the tenant.  YOU ARE RESPONSIBLE FOR ENSURING YOU PROVIDE CORRECT INFORMATION ABOUT YOUR LANDLORD OR IT WILL DELAY YOUR ASSISTANCE.  You will be notified at the email you provided on your application if and when your check is ready for pick up.  YOU ARE RESPONSIBLE FOR PICKING UP YOUR OWN ASSISTANCE CHECK.
