All sections must be completed before the application will be accepted by the Department of Community Development.
- A separate application is required for each project or activity. In addition, only typed applications via this Zoom Grants platform will be accepted. No paper applications will be accepted.
- All social service projects must adhere to income eligibility guidelines. For reference purposes they can be found on the page immediately preceding the application
- Prepare your program year 2024 application in a clear, comprehensive and concise manner. Remember to complete all sections and provide sufficient documentation to assure fair consideration of your application.
- When appropriate, provide personnel job descriptions with detailed salary and wage information.
- Section VIII will provide the Office with specific information that will be used to calculate Performance Measurements. Additionally, please be specific when completing these pages, as they will be used to formulate a scope of work for the project.
- Project leveraging is an important aspect of utilizing CDBG funds. Please complete Section VII using actual or prospective funding.
- Be certain your application has been signed by the Director of the organization.
- All applications will be reviewed by the Community Development staff to determine eligibility. If additional information is required you will be contacted by a representative.
- Each applicant is required to provide a Unique Entity ID (EIN) number. Existing registered entities can find their Unique Entity ID by following the steps here. New entities can get their Unique Entity ID at SAM.gov and, if required, complete an entity registration.
- Each applicant is required to provide an Employer Identification Number (EIN) which is used to identify a business entity. You can apply for an EIN number online at www.irs.gov
- Completed application(s) must be submitted using the link https://www.zoomgrants.com/gprop.asp?donorid=2410&limited=5630 to the Department of Community Development, no later than 4pm, March 28, 2025
